BOARD MEETING NOTICE The New Mexico Board of Examiners for Occupational Therapy will convene a Regular Board Meeting on Friday, May 3, 2013 at 11:00 a.m. The meeting will be held at the Regulation and Licensing Department located at 5200 Oakland NE, Albuquerque, New Mexico 87113. The Board may go into closed session pursuant to 10-15-1.H to discuss licensing and disciplinary matters as permitted by the Open Meetings Act. The public is welcome to attend. Persons desiring to present their views may appear in person or send their written comments, prior to the meeting, to the New Mexico Board of Examiners for Occupational Therapy office at P.O. Box 25101, Santa Fe, NM 87504. A final agenda for the meeting will be available at least 24 hours prior to the meeting and may be obtained by contacting the board office at 505-476-4940. REQUEST FOR PROPOSAL SINGLE AUDIT PUEBLO DE COCHITI The Pueblo de Cochiti (the Pueblo) is soliciting proposals from qualified certified public accountants to audit the Pueblo de Cochiti and Pueblo de Cochiti Housing Authority financial statements for the year ended December 31, 2012. Furthermore, the selected firm will have the option of auditing the Pueblo de Cochiti, Pueblo de Cochiti Housing Authority and Pueblo's for profit Corporation, Cochiti Community Corporation Development (CCDC) for the two subsequent fiscal years. These audits are to be performed in accordance with generally accepted auditing standards, the standards set forth for financial audits in the US General Accounting Office's (GAO) Government Auditing Standards (1988), the provisions of the federal single audit act of 1984 and U.S. Office of Management and Budget (OMB) Circular A-133, Audits of States, Local Governments, Non-Profit Organizations, and for Profit Organization. INVITATION FOR BIDS The SANTO DOMINGO TRIBAL HOUSING AUTHORITY (SDTHA) is requesting bids from General Contractors to stabilize six (6) roofs within the historic core of the Santo Domingo Pueblo village. All bids received shall be sealed and be legibly marked "Santo Domingo Housing Authority Roofing Stabilization Project." SCOPE OF WORK Project Description: This project consists of stabilization of six (6) roofs with various levels of deterioration to ensure weather tight functionality. Contract Documents, General Conditions of Contract, scope of work, Plans and Specifications will be included in the bid package. BID PACKETS Bid packets will be available at the SANTO DOMINGO TRIBAL HOUSING AUTHORITY (SDTHA) Office located on the Santo Domingo Indian Reservation on April 15, 2013 after 1:00pm MDT for a $50.00 nonrefundable fee (check or money order). SEALED BID RESPONSE TIME Bids shall be in a sealed envelope addressed to Kevin Esquibel, Project Manager of the SANTO DOMINGO TRIBAL HOUSING AUTHORITY (SDTHA). Sealed bids will be accepted in person at the offices of (SDTHA) no later than April 25th, at 2:00 p.m., Sealed bids may also be sent via certified mail to the (SDTHA) to the above address, Attention Project Manager and received no later than 1:00 p.m. April 25th, 2012. BID GUARANTEE A bid guaranty in the form of a satisfactory bid bond secured by acceptable sureties, in an amount equal to two percent (2%) of the bid shall be submitted with each bid. ASSURANCE OF COMPLETION The successful bidder shall be required to furnish and pay for satisfactory assurances of completion including but not limited to performance and payment bonds. SDTHA will consider alternative assurances for completion allowable within the guidelines in the instructions to Bidders. The assurance of completion shall be valued proportionately to the costs associated with the new construction project. INDIAN PREFERENCE Pursuant to Section 7(b) of the Indian Self-Determination and Education Assistance Act and 24 CFR 1000.52, SDTHA shall to the greatest extent feasible, give preference in the award of contracts to verifiable, qualified Indian organizations and Indian owned economic enterprises, as more particularly described herein. SDTHA shall also require contractors and subcontractors to provide preference to the greatest extent feasible by hiring qualified Indians in all positions. The IFB is open to all potential bidders; however, the award shall be made to the qualified Indian Enterprise with the most qualified responsive bid if that bid is (1) within the budget of the project, and (2) no more than 5% higher than the total bid price of the lowest responsive qualified bid. If there is no responsive bid from a qualified Indian bidder, then the award shall be made to the most qualified bidder. A labor and material itemization must be submitted with the bid. The (SDTHA) shall utilize this information for the purpose of identifying discounts, life-cycle expectancies and general bid balances. Failure to submit the cost itemization shall cause for automatic bid rejection. LICENSE - INSURANCE The successful contractor will be required to have a valid contractor's license in the state of New Mexico and carry General Commercial Liability, Workman's Comp, and Automotive Liability Insurance. The (SDTHA) reserves the right to reject any and all bids and waives any informality in bids wherever it is in the best interest of the (SDTHA). On September 26, 2005, KCEI was granted a license by the Federal Communications Commission to serve the public interest as a public trustee until October 1, 2013. Our license will expire on October 1, 2013. We must file an application for renewal with the FCC by June 3, 2013.
