Tuba City Regional Health Care Corporation (TCRHCC) invites Proposals from qualified firms to provide Architectural/Engineering Services to the Tuba City Regional Health Care Corporation, located in, Coconino County, Tuba City, Arizona. Invitations are open to all Indian and Non-Indian Owned Design Professionals. REQUEST FOR QUALIFICATIONS The City of Santa Fe is requesting interested Coating Applicators & General Contractors to submit Qualifications for the City's Hospital Tank Coating Application concrete water storage tank short term rehabilitation improvements, CIP 3039A, Project # 13/15/Q/B Qualification submittals from the coating applicators and general contractors will be received by the City of Santa Fe and six (6) copies will be delivered in a sealed envelope to the City of Santa Fe Purchasing Office at 2651 Siringo Rd., Building "H", Santa Fe NM 87505, until 2:00 P.M. local prevailing time, Thursday, March 14, 2013. Any Qualifications received after the deadline will not be considered. The City will select contractors qualified to bid on the project described below. Criteria for qualification will be based on the items A through C requested below. Proposing coating application companies must specify their New Mexico licensed GB-98 sub-contractor for the site work or proposing General Contractors must specify their coating application company. Failure to submit any one of the items below may result in disqualification. Several of the numbered items listed below are assigned a weighted value. Contractors who meet 80% of the total possible 100 points will be qualified to bid. No state or local preference will be applied to bid. Construction Phase services for this project includes the following: Install or apply waterproof coating similar to Xypex products or Vandex products to the interior surface of an existing concrete water storage tank. This also includes surface preparation per manufacturer recommendations. The applicators should be qualified or certified per the waterproof coating manufacturer requirements. Contractor should provide a 10-year warranty on the coating work. Clean all new cracks and apply water sealant products. Wire brush clean and paint all precast structural members (columns, beams and double tees) welded connections per project specifications. Notice to Proceed for construction will be on or about 04/15/13 and the substantial completion of the construction and the tank to be online by 08/03/13. Adherence to schedule will be paramount. Below is a list of the criteria that must be included within the qualifications submittal (8" x 11" report format - bound or stapled, and indexed). The weighted value of each item (if applicable) is also listed. A. Provide a general description of the firm. Explain the legal organization of the proposed firm or team. Provide an organization chart showing key personnel and job titles. (10 Points) B. Provide the following information: (20 Points) 1. List the New Mexico contractor licenses held by the firm and the key personnel who will be assigned to this project. Provide the contractor license number(s). Reference the appropriate licenses held, if needed. 2. Identify any contract or subcontract held by the firm or officers of the firm, which has been terminated within the last five years. Identify any claims arising from a contract which resulted in litigation or arbitration within the last three years. Briefly describe the circumstances and the outcomes. C. Submit the following information: (60 Points) 1. Identify at least three comparable projects in which the firm served as General Contractor or Sub-Contractor. Special consideration will be given to firms that have provided similar successful projects and coating application projects. Emphasis shall be placed on experiences on concrete water and wastewater tanks rehabilitation. For each project identified, provide the following: a. Description of the project (scope of work & location). b. Role of the firm as General Contractor. c. Project's original contracted construction cost and final construction cost. d. Construction dates (original contract & actual completion dates). e. Project owner. f.Two references (current names with telephone numbers per respective project). g. Percentage of work done in-house. h. Number of staff on each project. I. List total number of change orders, description & reasons per change orders. D. List of all City of Santa Fe projects where the firm provided general contractor services in the last five years, either completed or ongoing. (10 Points) The contractor's attention is directed to the fact that applicable Federal Laws, State Laws, Municipal Ordinances, and all rules and regulations of all authorities having jurisdiction over said item shall apply to the RFQ and will apply to the contractor that is selected.
