Hopi Tribe Wireless Equipment RFP Advertising V01A Wireless Network Equipment and Utility Poles RFP Advertisement Notice of a Request for Proposals (RFP) is available through the Hopi Tribal Government for three groups of equipment specified in separate Exhibits: Wireless Mesh Equipment, Microwave Backhaul Systems, and Wooden Utility Poles for delivery to the Hopi Reservation in NE Arizona. Vendors may bid on any or all of the three RFP Exhibits independent of each other with a purchase order being awarded by selection of the lowest bid price among compliant bids for each Exhibit separately. LEGAL NOTICE REQUEST FOR PROPOSALS CONSTRUCTION MANAGEMENT SERVICES FOR THE BLACK MESA PHASE 1A PIPE EXTENSION PROJECT The Albuquerque Metropolitan Arroyo Flood Control Authority (AMAFCA) requests competitive sealed proposals for Professional Services related to the construction of Black Mesa Phase 1a Pipe Extension Project. This project is currently in design and will be submitted to AMAFCA, Bernalillo County and the New Mexico Department of Transportation for approval. AMAFCA is requesting proposals for engineering services to provide construction engineering services and certifications for the project. Proposals shall be submitted in eight (8) copies no later than 2:00 p.m. on Monday, July 16, 2012 to AMAFCA, 2600 Prospect NE, Albuquerque, New Mexico 87107. Proposals shall be limited to six (6) pages, excluding cover letter and table of contents. A Selection Advisory Committee appointed by the AMAFCA Board Chairman will review proposals. Interviews may be conducted with some firms or individuals submitting proposals. However, proposals may be accepted or rejected without interviews. The fee shall be negotiated as part of the Professional Services Agreement. The selected firm will also be required to enter into a Memorandum of Understanding between AMAFCA and the Design Engineer. The scope of services, sample agreement, and sample memorandum of understanding may be picked up at the AMAFCA office, or requested by email via the AMAFCA website at www.amafca.org, beginning Monday, July 2. Project Description: The project is being designed by Wilson and Company, Inc. for AMAFCA. AMAFCA will be the contracting agency and desires to enter into an agreement for construction engineering services for the project, including certifications and close out packages to AMAFCA, the County and the New Mexico Department of Transportation. The project consists of installation of 17,000 linear feet of pipe, ranging in size from 36" to 60", primarily along the east side of the Gun Club Lateral. Some of the pipe will connect to outfalls from AMAFCA's three southwest valley dams, McCoy, Raymac and Don Felipe. The project will also rebuild the Gun Club Lateral, including all road crossings, turnouts, grade checks and maintenance roads. AMAFCA will maintain the pipe and maintenance road above the pipe upon completion. Estimated construction cost is $6,600,000. Construction time frame is September 2012 through April 2012. The project may be bid as three separate contracts, and the selected firm will manage all three contracts. Evaluation of Proposals: In order to assist the Selection Advisory Committee in making a recommendation to the AMAFCA Board of Directors, the format for the response (with the weighted value for evaluation of each criterion) shall be as follows: Item 15% General description of business, incorporating specialized design and technical competence of the business, including a joint venture or association, regarding the type of services required. State the amount of work that will be produced by a New Mexico-owned business within this state and amount and type of non-local expertise to be utilized. State proximity to or familiarity with the area in which the project is located. State familiarity with state, federal and local policies and procedures and with local construction/contracting practices. Item 225% Technical and managerial approach to the project. Item 340% Capacity and capability of the business, including any consultants, their representatives, qualifications. and locations, to perform the work, including any specialized services, within the time limitations. Describe organization and applicable experience of project team and planned participation of key team members on this project Item 410% Proposed schedule which reflects allocation of effort for each project team member per task. Describe cost saving measures to efficiently manage construction engineering services for the project. Item 510% Past record of performance on contracts with government agencies or private industry with respect to such factors as control of costs, quality of work and ability to meet schedules. List at least three relevant projects with references, the original construction schedule and actual time to complete the project, construction budgets, engineer's estimates for construction, change order costs and final installed costs. List construction phase engineering costs for the same projects, with budgeted costs and actual costs for inspection, testing and surveying services. State any relationships with other public or private clients which relate to this project. Item 65% The volume of work previously done for AMAFCA, which is not seventy-five percent (75%) complete, with respect to basic professional design and/or construction management services. List all AMAFCA work (with the exception of on-call assignments) and associated fees for the past three years. The objective is to ensure an equitable distribution of contracts among qualified businesses and to assure that the interest of the public in having available a substantial number of qualified businesses is protected, provided, however, that the principle of selection of the most highly qualified business is not violated. Item 75% Quality of submittal. The submittal shall also include the mandatory AMAFCA-specific Campaign Contribution Disclosure Form that is attached to and is made part of the Scope of Services. Failure to submit the AMAFCA-specific Campaign Contribution Disclosure Form shall be cause to reject the proposal. Tempur is requesting to install a calcium carbonate addition to the operations at the facility located at 12907 Tempur Pedic Parkway Albuquerque, New Mexico.
